The type of personal information we collect
We currently collect and process the following information:
- Personal identifiers:-
Your name, address, phone contact and email address.
- Information about you:-
Your GP contact, emergency contact, health information (relevant to you receiving your treatment).
How your personal information is collected.
The data is collected directly from you via electronic message or phone call and by face to face discussion. I also receive some data from NYR Organic (my online shop) when items are purchased.
Why your personal information is recorded.
- To allow us to contact each other should any changes or confirmations need to be made to your treatment or appointment time.
- For your safety should you take ill during your appointment, your emergency contact can be notified.
- Information regarding your health status, any mobility issues or any regular medication you may be taking is collected to enable any treatment adaptations to be made for your comfort and safety.
All information we hold is kept confidential with the only exception being:-
If any information you share with us leads us to believe you or a third party may be at risk of serious harm may be forwarded to the appropriate authority. Your permission to do this will be actively sought but referring your details may be done without this permission if you or a third party are genuinely believed to be at risk of serious harm.
Under the UK General Data Protection Regulation (UK GDPR), the lawful base we rely on for processing this information is:
Legitimate interests: the processing is necessary for your legitimate interests.
Purpose
- to enable communication between us regarding your appointment.
- to identify any contra-indications so that preventative action/s can be taken.
- To comply with our industry code of practice.
Necessity
- It is irresponsible and negligent for us to treat you without undertaking a health consultation beforehand because data from your consultation is used to design your bespoke treatment
Balancing test
- You reasonably expect the safest, most appropriate treatment for your needs and this is totally reliant upon the collection and processing of your personal data.
How we store your personal information
Your paper based information is stored in a locked filing cabinet in an upstairs office which is alarmed. The only person with authorised access to your information is Mrs Deborah Adams.
Your personal information is stored for 10 years (unless you request it be deleted or destroyed). Paper based records will be shredded or burnt after 10 years of inactivity.
Your contact details may be stored on Mrs Deborah Adams’ mobile phone which is code locked and only used by herself.
Your email address may be stored as a contact in MeTime Holistics email account to which Deborah Adams is the only person authorised for access.
Your data protection rights
Under data protection law, you have rights including:
Your right of access – You have the right to ask us for copies of your personal information.
Your right to rectification – You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.
Your right to erasure – You have the right to ask us to erase your personal information in certain circumstances.
Your right to restriction of processing – You have the right to ask us to restrict the processing of your personal information in certain circumstances.
Your right to object to processing – You have the the right to object to the processing of your personal information in certain circumstances.
Your right to data portability – You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
